Thank you for trusting your family veterinarian's referral to Fox Valley Veterinary Dentistry and Surgery for your pet's oral healthcare needs. We partner with your veterinarian to provide your pet with specialized dentistry and oral surgery care. We ask that you have preoperative bloodwork (including a complete blood count and biochemistry profile) completed by your family veterinarian prior to your appointment with FVVDS. Additional testing may be required for your pet as determined by Dr. Charlier and your family veterinarian.
Please complete the New Client Information Form and email it to our office at fvvds@sbcglobal.net or fax it to our office at 847-488-0705.
Most of Dr. Charlier’s consultation and recheck evaluation appointments are scheduled in the morning so that she has the remainder of the day to complete procedures. You have two options when scheduling an appointment with Dr. Charlier:
You may schedule your pet for a consultation only appointment with Dr. Charlier. During that appointment she will review your pet’s history from your family veterinarian and complete a thorough physical examination and oral examination. She will formulate the best treatment plan for your pet. If your pet requires a procedure you may schedule it at a later date.
OR
You may schedule your pet for a consultation and procedure appointment with Dr. Charlier. Dr. Charlier will meet with you and your pet in the morning. During that appointment she will review your pet’s history from your family veterinarian and complete a thorough physical examination and oral examination. She will then formulate the best treatment plan for your pet. Your pet will stay for the day to have the procedure completed. All of our patients are released the same day so they can sleep in their own ‘beds’. If you are traveling from a distance, arrangements can be made so that you may wait for your pet to recover from its procedure and be released from the hospital.
