We are committed to effective communication and team building to provide
quality patient care. We view our partnership with the referral veterinarian as
part of a triangle involving the client, referral veterinarian and Fox Valley
Veterinary Dentistry and Surgery. We strive to be an extension of your practice.
We require preoperative laboratory evaluation for all of our patients which will
include a minimum database of a CBC and chemistry profile which is to be
completed at your office prior to their appointment with FVVDS. Additional
testing may be requested for individual patients based on their needs.
All referral patients will be clients of FVVDS for their oral health care needs only
and will not be allowed to receive 'routine' health care during their visit to us.
Please complete a Veterinarian Oral Health Care Referral Form and fax or e-
mail it to us along with a copy of the laboratory evaluation and pertinent medical
